Disability

Disability insurance provides income replacement should you become disabled and unable to work due to a non-work-related illness or injury. Disability benefits help you financially, so you can focus on getting better. The company provides eligible associates with disability coverage at no cost, as shown below. Coverage is automatic; you do not need to enroll.

Our Short Term and Long Term Disability coverages, offered through Cigna, provide you with two options for how taxes will be paid should you need to use the benefit:

  • You can pay the premium post-tax from your paycheck. If you choose this option, you would receive the benefit tax-free if you become disabled.
  • Infoblox pays your premium each payroll however if you become disabled, you will pay tax on the benefits you receive.

Short Term Disability (STD)

Infoblox provides a STD benefit to all eligible employees. You will pay taxes on the premiums for this coverage so benefit payments received are non-taxable. In the event you have an extended disability and are unable to work due to an accident or illness, you may qualify for the following:

Benefits Begin

After seven calendar days, following an accident or illness.

Weekly Benefit

60% of your basic weekly earnings

Maximum Benefit

$3,460 per week

Maximum Benefit Duration

12 weeks

Definition of Disability

You are unable to perform the material duties of your regular occupation and you are unable to earn 80% or more of your covered earnings from working in your regular occupation.

Long Term Disability (LTD)

Infoblox provides a LTD benefit to all eligible employees. You will pay taxes on the premiums for this coverage so benefit payments received are non-taxable. In the event you are unable to work due to an accident or illness, you may qualify for the following:

Benefits Begin

After a 90-day waiting period of continuous disability from the date your disabling condition occurs.

Weekly Benefit

60% of your covered monthly earnings

Maximum Benefit

$15,000 per month

Maximum Benefit Duration

In the event you are unable to perform, this benefit could continue up to age 65, or your normal social security retirement age.

Definition of Disability

Unable to perform material duties and unable to earn 80% or more of indexed earnings from working.

Family Medical Leave Act (FMLA)

If you have been with the company for 12 months, you may be eligible for up to 12 work weeks of unpaid, job-protected leave per year under the Family and Medical Leave Act (FMLA). FMLA can be used when you are unable to work because of your own serioushealth condition, or because you need to care for your parent, spouse, or child with a serious health condition.  Leave may be taken all at once, or may be taken intermittently as the medical condition requires.
